Cygnus OA-6, being lofted on an Atlas V, is now scheduled for March 20, 2016; its launch window is 04:18-4:48 UTC (12:18-12:48 AM EDT).

This means that SpaceX CRS-8 is delayed to an undetermined date.

Also, SES-9 is scheduled for late February.

You have a few out of date Atlas tail numbers.  I didn't check them all to see which are correct, but here's the latest update chronologically:

OA-6:   AV-064
MUOS-5:  AV-063
NROL-61: AV-065
SBIRS GEO-3: AV-066
OSIRIS-REX: AV-067
WORLDVIEW-4: AV-062
GOES-R: AV-069
ECHOSTAR-19: AV-071
NROL-79: AV-068
AEHF-4: AV-072

Check out this link:

http://spaceflightnow.com/2016/02/08/astronaut-interview-30-years-after-the-hopes-of-a-west-coast-space-shuttle/

At the very end, it reads that NROL-45 on a Delta IV will launch at 11:39 UTC (3:39 AM PST).
Because the Falcon Heavy Test Flight was successful, it has inspired thousands of people to consider changing the future of space travel.

Spaceflight Now updated their launch schedule; MUOS 5, scheduled to launch on an Atlas V 551 (AV-063), is targeting a time within a period of 15:46 UTC to 19:46 UTC (11:46 AM EDT to 3:46 PM EDT). I understand my prediction might be wrong, but I predict the actual launch time is 17:46 UTC (1:46 PM EDT).

Is the new, specific launch date for InSight an indication that it isn't cancelled?

Also, if approved, would the launch site remain at Vandenberg for the 2018 launch window?

NASA could make a decision within a week.

most likely, however trajectory from the parking orbit might change a bit. there is also the slim possibility that a few SRMs might be added because of change in launch date, thus longer trip.
