Regarding the conjunction
TDLR: SpaceForce left the dummy object, which they were using as a placeholder for Endeavor, in their catalog and the real Endeavor and this "analyst sat" were predicted to collide.

Source of the FOIA document: SpaceX False Alarm Encounter with “Unknown Object” – April 2021 – The NASA Documents

Pretty amazing the details included in the FOIA result, NASA even included MS Teams chat logs. Some other Crew-2 tidbits from the document (some may already be discussed during livestream, I don't know since I didn't watch it):
1. Space Force is pretty embarrassed about the mistake, they called NASA to apologize, and they're considering this "an extremely high-visibility failure within the Space Force".
2. There's another collision related alert during countdown, some old Russia satellite is supposed to re-enter but didn't. Fortunately it turns out to be a non-issue, didn't affect the countdown.
3. A Draco sensor malfunctioned during flight, but they have 2 other redundant sensors on the thruster so didn't affect the mission
4. Apparently Draco firing is fairly loud, makes it difficult to sleep through it
5. Some FOD was released when Dragon is separated from Stage 2, not sure how big a deal this is, but it was mentioned
6. There's a mixup with phone line setup, such that SpaceX's flight surgeon couldn't call astronauts to provide PMC (Private Medical Conference?), so they had to kick everyone in Hawthorne mission control out so that the surgeon can use the mission control to talk to astronauts.

Rob Navias confirmed during an interview with the crew that a 2 hour fly around by the departing Dragon of ISS will occur if undocking/splashdown timing allows it.  The interview was on the ISSlive streaming feed but will not be released till next week.

Nice to see the details of this mistake were finally made public.  Bravo to Mr Greenewald and everyone else who went through the effort of putting in the FOIA request(s) and got them released. Where were the other media outlets? NASA/Space Force should have released them without being forced to...

Cross-publication on Crew-2. (taken from the last update before the launch of Crew-3)
This is a summary of the most important points made at the press conference:
5.   Crew-2 astronauts are not allowed to use the toilet on Crew Dragon Endeavour and will not be able to use it during their return so they will wear "underwear".
8.   If the Crew-3 mission launch is delayed, the Crew-2 crew return is also delayed (currently scheduled for November 4).
9.   The second launch opportunity for Crew-3 is Wednesday, November 3 at about 06:10 UTC; if launch occurs on Wednesday, Crew-2's return will be at least November 7.
