According to reports 2nd stage separation and 3rd stage ignition did occur as planned, but then the stage experienced attitude control problems (RCS problems?). The stage reached 340 km in altitude but the final velocity was only 6.X km/s, not enough to enter orbit.

My Hypothesis based on NSF posts: motor or counter weight spin up for Rhumb line control with a thruster section (trim module per previous posts of NSF) for terminal phase of flight, despin, orientation for SC Sep, and CCAM / back-off sequence.
