On Wednesday, the executive director of Roskosmos, Sergei Krikalev, said that in 1986 (the Soyuz-U booster rocket was being launched) a similar situation occurred: the first stage hit the second stage, as a result of which the flight stopped.
Interesting, I hadn't come across an explanation for that failure before.

EDIT: Upon closer inspection in Google Earth, you can see the landing point is only ~3 km away from the river! Meanwhile, the fairing fragments (presumably containing the Orbital Module) fell just 600 m away from the A17 highway.

Thanks for the slides, especially the last one shows that there is much more into manned launches than just a spacecraft and a launch vehicle, as all these assets and people are on high alert during every single launch!
