  I haven't seen complex 16/2 listed for a launch since the last Soyuz-U launch from Plesetsk in 2012.

• The stack of Fregat-M and Glonass-M separated from the third stage. It will take several hours for Fregat to deliver satellite to target orbit.

  yes. 43/3 is in upgrades for Soyuz 2.1 Family excluding Soyuz 2.1v
  yes. 16/2 is in upgrades for Soyuz 2.1v only. This will become dedicated pad for Soyuz 2.1v and its fully decommissioned backup pad is disused pad 41/1. currently 41/1 will not be rebuilt unless a long term need/demand arises.
