Do we have any data on how many times individual examples of the 18 Shuttle/ISS EMUs have been used on EVA? 

Likewise Orlan suits on the ISS?

https://oig.nasa.gov/docs/IG-17-018.pdf has some info but nothing as detailed as what you describe; some info could be inferred.
