Cross posting from Soyuz MS-16 page posts by Olaf  and jcm

https://www.roscosmos.ru/28064/

The Russian members of the main crew of the Soyuz MS-16 manned spacecraft, Roscosmos cosmonauts Nikolai Tikhonov and Andrei Babkin, will be replaced by backups for medical reasons.

The positions of the commander and flight engineer of the Soyuz MS-16 spacecraft will now be taken by the cosmonauts of Roscosmos Anatoly Ivanishin and Ivan Wagner. The changes will affect only the Russian part of the crew, NASA astronaut Chris Cassidy continues to prepare for the flight in accordance with the established plan.

The launch of the Soyuz-2.1a launch vehicle with the Soyuz MS-16 manned spacecraft and members of the long-term expedition ISS-63 is scheduled for April 9 this year from launch pad No. 31 of the Baikonur Cosmodrome.

According to a Vedomosti source involved in the ISS program, the crew was replaced due to an eye injury that the commander received while accidentally stumbling into a branch while walking.

Comment by me, ddspaceman...

This is not the first time Nikolai Tikhonov  has gotten close to flight and had the rug pulled out from under him.  He must be frustrated!
From WikiPedia:  https://en.wikipedia.org/wiki/Nikolai_Tikhonov_(cosmonaut)

He was selected as a backup flight engineer for Soyuz MS-02. He was made a prime crew member of Soyuz MS-04 but, due to Russian budget cuts, the crews were changed and Tikhonov was then slated to make his first spaceflight on the Soyuz MS-10 spacecraft. He was again removed from the manifest due to delays in launching the Russian Nauka module.[1][2] He was scheduled to fly into space as commander of the Soyuz MS-15 mission, although he was removed for a third time following the aborted launch of Soyuz MS-10, and the subsequent crew changes that followed. He is was scheduled to finally make his first flight onboard Soyuz MS-16 in 2020, but ended replaced due medical reasons.[3]

Some interesting information on the new crew
Quote
The possibility of an extended period with three residents on the station — and just one U.S. astronaut — prompted managers to train the original Soyuz MS-16 crew on systems inside the U.S. segment of the outpost.
Tikhonov trained to use NASA’s spacesuits in case he needed to join Cassidy on a spacewalk performing repairs or maintenance outside the space station. And Babkin was certified to operate the station’s Canadian-built robotic arm.
Megan Sumner, a NASA spokesperson, said Wednesday that Ivanishin and Vagner will not travel to the Johnson Space Center in Houston for additional training before their April 9 launch. Cassidy will head to Russia later this month to join his new Russian crewmates for final training and familiarization with the Soyuz spacecraft, before heading to the Baikonur Cosmodrome in late March for launch preparations.
Cassidy was previously a crewmate of Ivanishin and Vagner on the Soyuz MS-16 backup crew before he was elevated to the prime crew last year to replace a Japanese astronaut. During that time, Sumner said Cassidy conducted U.S. spacesuit and spacewalk training with Ivanishin, including training inside NASA’s Neutral Buoyancy Laboratory at the Johnson Space Center in Houston.
