Onboard Anomaly during the Z40 second stage burn. Official information pending formal announcement. DDO announced Anomaly with the launcher. Loss off thrust resulting 3.30km/s maximum velocity and maximum apogee 110km at 500km downrange. Under pressure event on Z40

No visible boom shown on stream nor observed via very very preliminary information however flight would have been terminated by the RSO upon going out of bounds anyways to ensure the entire vehicle destroyed.

The end of 2nd stage thrust was clearly visible on the video (of course could be occluding cloud cover or whatever).

They always handle launch failures so badly on their webcasts. They put up conflicting graphics (graph of projected and actual trajectory showing failure, next to an animation showing the vehicle continuing happily on) and take a large amount of time to confirm the obvious.

Edit, yeah, pretty trivial complaint. But respect for your audience, including the ordinary people whose taxes fund your efforts, should be a thing.

Second stage burn.  Note that the performance plot begins to visibly diverge from the plotted line circa T+03:50.  The CGI simulation continues, but the third stage burn, apparently, did not occur.
