It's an upper stage for a Space Shuttle: it would sit on the back of a manned flyback booster in place of the orbiter. This was in the early days of the program when the Shuttle was to have two manned stages. By putting the solids off to the sides, rather than evenly spaced around, the stage would fit comfortably alongside the booster.

Here we go, from a McD report from 1971. Poseidon SLBM motors don't often show up in proposals for NASA boosters.

the only other proposal for Poseidon SLBM for Rocket,
i know, was a Titan I with Poseidon as Upper stages around 1957

Interesting concept for a Saturn V.  Looks like its an S-IVB version for a Saturn V given the helium storage tanks.
