Why doesn't the elevator go all the way up to the level for the crew access arm? Why go _almost_ all the way, then have a one-storey flight of stairs?

That level didn’t exist when the elevator was built. SpaceX added a “half level” for the CAA.

Shuttle era, but the Shuttle FSS also has heritage as the Apollo Launch Umbilical Tower if I remember right. Not sure if that includes the elevator.

The Dragon looks relatively roomy still. Do they have any cargo bags internal to the module or are they under the seats somewhere, hidden from view?
