Ben Cooper's site still says this is launching from SLC-40. But interestingly, it also says Transporter-6 is still planned for a mid-December launch without specifying the pad, so maybe T-6 is going to launch from LC-39A?

Next Spaceflight says that Starlink 4-37 will launch from Pad 39A NET December 17th. AFAIK, this was updated within the last ~24 hours.

I should have received a standalone Operations Completed cancellation notice from the NGA for HAKUTO-R.  Instead, I received this cancel-and-replace NGA notice that cancels the HAKUTO-R notice and replaces it with this information for what appears to be a Starlink launch.  But not on December 17.  The primary day is December 16, at almost the same time as O3b mPOWER 1-2.

Might SpaceX be hedging their bets by setting up notice for a December 16 Starlink launch if something between the 11th and (perhaps) the 15th delays the mPower launch?

If the mPower launch is not delayed, then this Starlink launch proceeds on the 17th.

(Also noting there are 3 Falcon 9 launches scheduled 15th-16th-17th.)

NextSpaceflight (Updated 12 December) now also reporting 16 December, 22:00 UTC. So yes, it does look like SpaceX is targeting back-to-back launches from both of their East Coast launch pads only 39 minutes apart. (Which is CRAZY)

Next Spaceflight has moved it...6 minutes to the left with the launch time now at 21:54 UTC.

I think we will have to wait to see if SpaceX really plans to have both launches on the same hour, it does seems a bit tough to pull this out.

Didn’t Elon once make a long-term goal way back then to have two Falcon 9 launches occur on the same hour?

If they want launch Starship with Tanker flights back to back. This would be good test compagine.
