Don't know if these exist somplace else on the site, but Buzz Aldrin recently tweeted his Apollo 11 travel voucher and the Apollo 11 custom form.

Yes the #Apollo11 crew also signed customs forms. We brought back moon rocks & moon dust samples. Moon disease TBD

This is different than the towing bill Grumman gave Rockwell after Apollo 13 and some of the other tongue and cheek Apollo vouchers that have been posted.

The travel voucher is a real document, but the customs form is just one custom official's attempt at a souvenir.
As researched by NASA's History Office, the Johnson Space Center history office and the Customs Dept. history office, the original form can be traced back to one agent's attempt to get the Apollo 11 astronauts autographs. As the crew was in quarantine, he sent the form to NASA Headquarters. The form was likely not an official record but rather one person's memento.
