Does anyone know if this is GT-225GM? This launch was delayed from February this year.

Yup, that was it.
For posterity, "The missile was pulled from the 341st Missile Wing’s complex and the test" was performed "by a crew from Malmstrom AFB, according to Air Force Global Strike Command.

Here's the video. It was GT-228GM.

This was also a missile pulled from the 341st MW at Malmstrom AFB.
https://theelectricgf.com/2018/11/07/malmstrom-missile-used-in-test-launch/

As for silo number, they have used LF-04, 09, and 10 in recent years.  Can anyone tell from the topography of the press image which silo was used?

Here's the launch video. Are they using a new designation for the flight number? The new number seems to be FTU-1 (Flight Test Unit).

FTU1 is a special test not in the regular GTnnnGM series. It is Flight Test Unit 1 of the Mk 21 RV's new fusing system (or "fuze" as they call it)
