Did they ever determine which antenna cover was missing? Any photos?

This was the 9th EVA of Fyodor? Is he, thus, tied for third place for most EVAs ever with Jerry L. Ross? And has surpassed him in total EVA time at 59:09min?

Yes, it was WAL-6 on the fwd end of Zvezda. Apparently they had already figured that out from looking at it from inside the station and didn't even bother to go visit it, if I understood the PAO correctly.
