Polyot released an image of their factory, showing that they don't just build the Cosmos 3M anymore. Somewhere in there are pieces of Protons.

It seems that LV for AM4 was shifted many times. It was 93525/99524 indeed but in April, 93523/99525 in May and 93521/99522 actually. Sorry for the error.

 14S75 appears to be a generic term for any Proton payload fairing manufactured by Khrunichev, including the four Briz‑M fairings and the Blok‑DM fairing.
 14S326 is the 465 mm high metal payload adapter attaching the payload to the upper stage either Briz‑M or Blok‑DM; also used by Ekspress‑AM33.

 KAZSAT and Ekspress-AM3 were to have been delivered by Proton-M, featuring a Briz-M with 11D458M settling and impulse adjustment thrusters. The launch was scheduled for December 2005. NIIMASH delivered the 1st set of 11D458M during June 2005.
 Tests on 1st stage engines RD-275M were also completed by June 2005.
 However the launch of Ekspress-AM3 was brought forward by six months to June 2005, and KAZSAT was delayed after Monitor-E to June 2006.
